Application Questions:
How many years of dental hygiene experience do you have?
Are you skilled in co-diagnosis?
Are you local anesthetic certified?
Are you trained in adjunctive therapies, such as: laser, nitrous oxide, etc.?
What has your experience been with non-surgical periodontal therapy?
Are you willing to undergo a background check, in accordance with local laws/regulations?
Are you willing to provide 1-3 professional references upon interviewing process? Please include: person's name, title, email or phone number, and explain your relationship (former boss, current coworker, etc).
Requirements
Valid Wisconsin state license to practice as a Dental Hygienist
